BOARD OF HEALTH OF THE TOWNSHIP OF CHATHAM

MORRIS COUNTY, NEW JERSEY

 

NOTICE, is hereby given that at a meeting of the Board of Health of the Township of Chatham, in the County of Morris, New Jersey, held on July 8, 2008, the following ordinance was introduced and passed on first reading and that said ordinance will be considered for final adoption after public hearing at a meeting of the Board of Health to be held on Tuesday, August 12, 2008 at the Municipal Building, 58 Meyersville Road, Township of Chatham, New Jersey at 7:30 p.m. or as soon thereafter as the hearing may be conducted, at which time all persons who may be interested will be given an opportunity to be heard concerning such ordinance.  During the week prior to and including the date of said meeting copies of the ordinance will be made available at the office of the Board of Health, in the Municipal Building to members of the general public who may request the same.

ORDINANCE BH-1-08

 

 AN ORDINANCE AMENDING THE REGULATIONS OF THE BOARD OF HEALTH OF THE TOWNSHIP OF CHATHAM SETTING THE FEES FOR THE ISSUANCE OF CERTIFIED COPIES OF VITAL STATISTIC RECORDS

 

WHEREAS Boards of Health are required under the provisions of N.J.S.A. 26:8-1 et seq., to provide for the registration of vital statistics and to issue certified copies of such records, upon request;

BE IT ORDAINED, by the Board of Health of the Township of Chatham in the County of Morris and State of New Jersey, on this 8th  day of July, 2008, as follows:

 

CHAPTER 1. VITAL STATISTIC FEES

 

1.               The fees for the issuance of certified copies of vital statistics records, including but not limited to death records, marriage records, birth records, certificates of domestic partnership, and civil union certificates, shall be $10.00 (ten) dollars for the first page and $5.00 (five) dollars for each and every page thereafter. 

2.               Once a vital record is amended, the correction form becomes part of the original record. Any time a copy of the vital record is issued, a copy of the correction form must also be issued.  Accordingly, no additional fee shall be charged for a copy of a vital statistic correction form when a copy of the vital record is issued.

3.               This Ordinance shall take effect after final publication in accordance with the laws of the State of New Jersey.
